	Vital Records Information

	TO OBTAIN BIRTH, DEATH AND/OR MARRIAGE RECORDS No personal checks accepted

	Birth Records, Death Records, Marriage Records
	Birth certificates are electronically submitted to the State Department of Health from the hospital. Certified copies of birth certificates can be issued currently back to 1905 for $15.00 each. Certified copies of death certificates and marriage licenses are $15.00 each. Download Vital Record Application Instructions
